血清非高密度脂蛋白—胆固醇的水平及其临床意义被引量:3
2007年
目的探讨血清中非高密度脂蛋白-胆固醇(nHDL-C)的水平和冠心病(CHD)的危险水平界值。方法根据血清总胆固醇(Tc)和高密度脂蛋白-胆固醇(HDL-C)的测定值计算非高密度脂蛋白-胆固醇(nHDL-C)之值;采用百分位数法获得正常参考范围;并对其男女进行比较分析。同时观察93例CHD患者的nHDL-C的值,求出阳性率。结果血清nHDL-C理想水平为1.97~3.77mmol/L;其中男1.99~3.89mmol/L;女1.89~3.67mmol/L。参考值为1.97~4.63mmol/L;其中男1.99~4.65mmol/L;女1.89~4.21mmol/L。nHDL-C随年龄有上升趋势,60岁后有下降趋势;且男性高于女性。nHDL-C≥4.65mmol/L为预测CHD发病危险性的界值。93例CHD患者阳性率为69.9%。结论血清nHDL-C的理想水平小于3.77mmol/L;男女应使用自己的nHDL-C的理想水平和参考范围;nHDL-C≥4.65mmol/L为诊断CHD的界值,其阳性率高达69.9%优于其它血浆脂蛋白(Lp)单项;具有较高的临床价值,且易于获得。

定值曲线法测定凝血酶原时间国际标准化比值被引量:1

目的:利用正常血浆和定标血浆,建立一种准确可靠、易于实验室开展的凝血酶原时间国际标准化比值检测方法。方法:根据准确度的传递,将被定值的正常血浆和定标血浆同时用称量法分别作6个不相同的稀释度稀释后上机进行检测。将待定值血浆PT值(X轴)与稀释度1/A(Y轴)绘制“校标”曲线;根据定标血浆测得的PT值,用内插法查得相对浓度1/A(X%)与计算相对浓度(Y%)绘制曲线,并进行回归处理得到INR的定值曲线;根据定值曲线绘制PT测(Y轴)与INR测(X轴)INR应用曲线。结果:血浆INR值与PT值有良好的线性关系(r=0.997),同一试剂本方法检测质控血浆INR值,重复性良好(CV<2%);不同试剂测定血浆INR值,结果显示本方法优于公式计算法,其结果与手工结果有很好的相关性(r=0.99),同时比仪器法更接近手工检测法(t检验P值:0.021对0.077)。结论:本方法在检测抗凝治疗患者血浆INR值时,干扰因素少,结果准确可靠,可能更接近待检血浆的INR“真值”。

Objective To understand three big and familiar interference things are concrete interference circumstances inbiochemistry examine and it's the dest eliminate way.Methods Three specimens of the serious hemolytic and the serious jaundice and the serious milk blood are different.diluted by physiology brine ( 0.9%NaCL ) and distilled water ( DH2O ) Every kind of items is examined under establishing the blank of sample and reagent , and analysis comparison result.To put a normal specimen into adopt artificial hemolytic.To determine potassium ( K ) and hemoglobin ( Hg ) in the serum of different hemolytic degree , and them analysis results. Results On OLYMPUS AU-400, when TG>37.04mmol/L and CHO>25.59mmol/L, T-BIL>398.7~tmol/L and D- BIL>60.81μmol/L and Hg>51.2g/L, the results can not be detected.Most items have comparison relation between It' s 1 : 3 to 1 :8.But TBA Lpa is 1 : 4 to l : 6 and Ca P is l : 1.5 to l : 2.5.Determining results have not comparison relation when the dilute times are larger than 1 : l0 and smaller than 1 : 3.Both results have not significant difference within having comparison relation after the specimen is diluted by 0.9%NaCl and distilled water ( P>0.05 ).But Lpa has significant difference( P<0.05 ).On BeckmCX3o The results an of Na and Cl have significant difference after the .specimen is diluted by 0.9% NaCl and distilled water ( P<0.05 ) .When the serious jaundice and the serious milk blood is diluted by distilled water, results of K Na CI CO2 BUN have not significant difference ( P>0.05 ) .So is Na Cl CO2 BUN Ca Cre for the serious hemolytic specimen .But Ca Cre and K have significant difference ( P<0.05 ) .The relativity of Hg and K are good in serum after a normal specimen is artificial hemolytic (r=-0.9999) .Comparing with determining results and formula calculating results of K have not significant difference ( P>0.05 ) .Conclusion Three kinds of familiar interference substances havs interference for biochemistry examining.Different instrument and reagent and methods cause different
