Analysis of Seizure Risk Factors after Allogeneic Hematopoietic Stem Cell Transplantation: A 8 Case Report and Literature Review被引量:2

The clinical characteristics of patients with seizures after allogeneic hematopoietic stem cell transplantation (allo-HSCT) were analyzed. A total of 8 cases of seizures after allo-HSCT were investi- gated. Clinical data of these cases were studied retrospectively. Of 159 cases subjected to allo-HSCT, seizure occurred in 8 cases during 29-760 days after transplantation, median survival time was 46 days, and there were 6 cases of tonic-clonic seizure. The incidence of seizure after matched unrelated HSCT was higher than that after related HSCT (P=-O.O17). Of 7 cases treated with cyclosporine A (CsA), 4 cases obtained high blood levels of CsA. In addition, hyponatremia was diagnosed in 5 cases. Abnormal electroencephalogram and brain MRI findings were found in some cases. During 20 days after seizure, 2 cases died due to infection and graft-versus-host disease (GVHD), respectively. It was suggested that multiple factors are associated with seizures after aUo-HSCT. Rapid identification and correction of the causative factors are very important to prevent permanent central nervous system damage and reduce the mortality.

细胞因子诱导杀伤细胞活性影响因素的研究进展

细胞因子诱导杀伤(CIK)细胞是一种过继免疫细胞,因其具有来源广泛,体外易于扩增,非MHC限制性,GVHD效应弱,副作用小及对多种肿瘤具有强大的杀伤活性等优点而成为近年来过继免疫治疗的焦点。但是关于如何进一步提高CIK细胞的增殖、杀伤活性,为患者带来更多益处?这一问题一直为研究者所关注。现从CIK细胞的培养体系,CIK细胞与其他细胞间的相互作用,改变CIK特异性及其他因素等方面来讨论影响CIK细胞活性的因素。

Effects of Dendritic Cell-activated and Cytokine-induced Killer Cell Therapy on 22 Children with Acute Myeloid Leukemia after Chemotherapy被引量:8

The efficiency of dendritic cell-activated and cytokine-induced killer cell(DC-CIK) therapy on children with acute myeloid leukemia(AML) after chemotherapy was investigated. Mononuclear cells were collected from children achieving complete remission after chemotherapy,cultured in vitro and transfused back into the same patient. Interleukin-2(IL-2) was injected subcutaneously every other day 10 times at the dose of 1×106 units. Peripheral blood lymphocyte subsets and minimal residual disease(MRD) were detected by flow cytometry. Function of bone marrow was monitored by methods of morphology,immunology,cytogenetics and molecular biology. The side effects were also observed during the treatment. The average follow-up period for all the 22 patients was 71 months and relapse occurred in two AML patients(9.1%). The percentage of CD3+/CD8+ cells in peripheral blood of 15 patients at the 3rd month after DC-CIK treatment(36.73%±12.51%) was dramatically higher than that before treatment(29.20%±8.34%,P〈0.05). The MRD rate was 〉0.1% in 5 patients before the treatment,and became lower than 0.1% 3 months after the treatment. During the transfusion of DC-CIK,side effects including fever,chills and hives appeared in 7 out of 22(31.82%) cases but disappeared quickly after symptomatic treatments. There were no changes in electrocardiography and liver-renal functions after the treatment. MRD in children with AML can be eliminated by DC-CIK therapy which is safe and has fewer side effects.

慢性淋巴细胞白血病的临床免疫表型分析被引量:7

目的评价慢性B淋巴细胞白血病(B-CLL)患者骨髓或外周血白血病细胞的免疫表型在B-CLL诊断、治疗和预后中的应用价值。方法收集26例B-CLL患者外周血或骨髓标本,应用多参数流式细胞术进行免疫表型测定。结果 26例B-CLL患者外周血淋巴细胞比例明显增加,典型的B-CLL高表达CD19、CD20、HLA-DR、cCD79a、CD5、CD23,而CD3、CD4、CD8、CD2各项指标均极度减低。结论测定B-CLL患者外周血或骨髓白血病细胞的免疫表型对于B-CLL的诊断及预后评价有重要意义。

目的观察树突状细胞(DC)与细胞因子诱导的杀伤细胞(CIK)共培养细胞(DC-CIK)过继免疫治疗儿童急性白血病的作用及安全性。方法回顾分析2005年5月-2011年5月本院确诊经联合化疗后达完全缓解的47例急性白血病患儿临床资料,观察DC-CIK过继免疫治疗过程中细胞采集、细胞培养、细胞回输的安全性,随访过程中通过定期检测其血常规、肝肾功能、心电图等来评估其远期安全性;DC-CIK过继免疫治疗后通过定期进行患儿骨髓细胞学、微小残留病(MRD)检测,初步评判其疗效。结果47例患儿接受DC-CIK治疗后44例患儿持续完全缓解(CCR),CCR率达93.62%;治疗前MRD阳性的9例患儿,8例治疗后转阴。47例患儿共接受DC-CIK过继免疫治疗61例次,不良反应轻微,单个核细胞采集术后局部有轻微疼痛,且均在术后2 h左右自行缓解。细胞培养过程中进行3次病原体检测,仅1例次培养出毛霉菌。细胞回输过程中23例次出现发热,10例次出现皮疹。中位随访41(5~72)个月未见心、肝、肾等重要器官功能损害,未见感染性疾病、第二肿瘤等的发生。结论 DC-CIK过继免疫治疗儿童急性白血病操作流程简便易行、安全性好,获得了满意疗效。

自体DC-CIK过继免疫治疗儿童急性白血病的疗效观察被引量:7

目的回顾性评估化疗联合自体共培养树突状细胞、细胞因子诱导的杀伤细胞(co-cultured dendritic cellswith cytokine-induced-killer cells,DC-CIK)的过继免疫细胞治疗对儿童急性白血病的临床有效性和安全性。方法将体外培养扩增并质检合格的自体DC-CIK细胞回输给47例急性白血病患儿,观察输注期间的不良反应,治疗结束后定期复查患者生存情况、微小残留病(minimal residual disease,MRD)水平及淋巴细胞亚群。结果 47例患儿顺利完成61例次DC-CIK回输治疗,中位随访41(5~72)个月。38例MRD阴性患儿中36例处于持续完全缓解(continuous completeremission,CCR),其中1例伴乙肝大三阳化疗不耐受者,治疗后获CCR,同时大三阳转化为小三阳,肝功能恢复正常;9例MRD阳性患儿回输治疗后8例转阴。治疗后患儿外周血淋巴细胞亚群检测显示CD3+CD8+细胞比例显著增加。结论 DC-CIK过继免疫治疗对儿童急性白血病微小残留病有明显的清除优势,安全性好,是继化疗、造血干细胞移植后又一新的治疗白血病的模式。

肿瘤免疫逃逸是肿瘤细胞通过多种机制逃避机体免疫系统识别和攻击,从而在体内继续生长和增殖。在肿瘤形成早期,免疫系统能够识别肿瘤细胞表达的特异性抗原决定簇,从而清除肿瘤细胞;但一些弱免疫原性的肿瘤细胞未被免疫系统完全破坏而潜伏在体内;最终这些肿瘤细胞能克服免疫系统的抑制作用,形成免疫耐受的微环境,从而进行性生长。在肿瘤免疫逃逸过程中,肿瘤血管发挥着重要的作用;它不仅是肿瘤细胞和免疫细胞间的第一道物理屏障,而且组成肿瘤血管的内皮细胞表型和功能特性改变也与免疫逃逸有着密切关系。内皮细胞也是免疫反应中最早被T细胞识别的靶细胞,同时对肿瘤免疫反应有着重要的调控作用。因此,本研究就肿瘤内皮细胞介导肿瘤免疫逃逸的机制进行重点介绍。
