心室不应期离散度及激动周期变异与心室颤动诱发的关系

目的 研究不应期离散度及激动周期变异与心室颤动(VF)诱发的关系.方法 采用6只开胸犬,在其左心室前壁心外膜缝上一块含有12×12个单极电极(电极间距2mm)的电极板.采用连续起搏方法,分别记录300 ms、200 ms及诱发VF或失去1:1心室夺获前不应期及激动周期.通过不应期标准差及其变异系数测定不应期离散度,激动周期差值的绝对值之和及其均值评估激动周期变异大小.结果 有5只犬通过此刺激方案诱发VF,VF的诱发率为83.3%.与300 ms起搏时相比,200 ms起搏时心室不应期离散度及激动周期变异差别不明显,差异无统计学意义;与300 ms及200 ms起搏时相比,VF发生或失去1∶1心室夺获前心室不应期离散度及激动周期变异显著增大,差异有统计学意义.结论 心室不应期离散度及激动周期变异与VF诱发相关,两者变异增大易于促发VF.

粒细胞集落刺激因子对家兔心肌缺血再灌注损伤的急性保护作用研究被引量:1

目的 探讨静脉用粒细胞集落刺激因子(granulocyte colony-stimulating factor,G-CSF)对家兔缺血再灌注心肌是否有急性保护作用,既能否增加心肌保护性蛋白磷酸化-丝氨酸/苏氨酸蛋白激酶( phosphorylation-Akt,p-Akt)的表达及是否具有抑制室性心律失常作用.方法 14只家兔随机平均分为2组(G-CSF组和对照组,n=7).所有家兔均开胸结扎冠状动脉左前降支30min,再灌注4 h(G-CSF组再灌注同时静脉泵G-CSF 10 μg/kg维持30 min,对照组静脉用生理盐水10 mL/kg维持30 min).利用60道袜套状电极进行双心室整体心外膜电生理标测.两组分别于基础状态下,缺血时及再灌注4h后通过程序性电刺激诱发心室颤动(VF),并测定除颤阈值(defibrillation threshold,DFT).通过快速傅里叶转换技术测定每个标测电极的VF激动频率.应用Curtis-Walker评分系统评价两组再灌注4h内室性心律失常发生情况.ELISA法测定血清肿瘤坏死因子α(TNF-α)和白细胞介素-10(IL-10)的浓度,蛋白质印迹(Western Blot)测定缺血区心肌p-Akt的表达水平.结果 所有家兔均可通过程序性电刺激诱发出稳定VF.与对照组相比,G-CSF显著降低了再灌注室性心律失常分数[(6.3±1.5)对(2.3±2.6),P=0.038].两组间基础状态下、缺血及再灌注时DFT及VF激动频率差异无统计学意义(P>0.05).两组再灌注4h后VF激动频率较基础状态及缺血时降低,差异有统计学意义(P<0.05).G-CSF不增加血清TNF-α及IL-10的浓度,但可增加缺血区心肌P-Akt的表达.结论 静脉用G-CSF未改变VF激动频率及缺血再灌注DFT,但是G-CSF通过增加缺血区心肌组织中p-Akt的表达可减少心肌损伤,减少再灌注心律失常.研究结果提示静脉使用G-CSF对缺血再灌注心肌具有一定的急性保护作用.

Ibutilide decreases defibrillation threshold by the reduction of activation pattern complexity during ventricular fibrillation in canine hearts被引量:1

Background Ibutilide has been commonly used for pharmacologic cardioversion of atrial fibrillation and flutter in clinical settings. The objective of this study was to investigate the effects of ibutilide on the defibrillation threshold (DFT), restitution properties, dispersion of refractoriness and activation patterns during ventricular fibrillation (VF). Methods Ibutilide was administrated intravenously in six open-chest beagles. Before and after the drug administration, 20-second episodes of VF were electrically induced and recorded with a 10×10 unipolar electrode plaque sutured on the lateral epicardium of the left ventricle. DFT and VF activation patterns, including type of epicardial activation maps, VF cycle length (VF-CL), conduction velocity, wavelength (WL) and reentry incidence, were measured. Restitution properties and dispersion of refractoriness were estimated from activation recovery intervals (ARI) during pacing. Results Compared to baseline, ibutilide markedly decreased the DFT by 31% ((491±14) V vs. (337±59) V, P 〈0.01). The drug significantly reduced the maximal slope of the restitution curve (1.34±0.08 vs. 0.76±0.06, P 〈0.01) and its epicardial dispersion (0.36±0.09 vs. 0.21±0.06, coefficient of variation, P=0.03). The dispersion of refractoriness was enhanced at the pacing cycle length of 300 ms to 160 ms by ibutilide. The drug significantly increased the VF-CL ((96±19) ms vs. (112±20) ms, P 〈0.01) and the WL ((41±9) mm vs. (52±14) mm, P=0.02) during VF, and reduced the reentry incidence by 25% (0.08±0.02 vs. 0.06±0.02, P 〈0.01). In the epicardial activation maps, ibutilide significantly reduced the percentage of more complex activation maps during VF. Conclusions Intravenous ibutilide significantly decreased the DFT. It might be due to reduction of activation pattern complexity during VF.

ROLE OF PURKINJE FIBERS IN MAINTENANCE ANDTERMINATION OF LONG-DURATION VENTRICULARFIBRILLATION

Long-duration ventricular fibrillation (LDVF) often occurring out-of-hospital has been presented several minutes before electrical shocks. It is important to understand the mechanism by which LDVF is maintained and defibrillated. Purkinje fibers (PFs) have been demonstrated to play a key role in the onset of certain types of ventricular fibrillation. In this review, we discuss the electrophysiological difference between PFs and working myocardium, and the role of the PFs in the maintenance and termination of LDVF.
